Shoulder Pain From Using a Computer Mouse: 3 Real Causes and How to Fix Them in 2026

If your shoulder aches by mid-afternoon but only on your mouse side, you don't have a mystery injury. You have a positioning problem. Here's the mechanism, because it explains everything that follows. Your arm is heavy — each one is roughly five to six percent of your body weight. The moment your hand leaves a supported surface to reach, hover, and click, your shoulder muscles switch on to hold that weight up. They don't fire hard. They fire constantly, for hours, and that low, unrelenting load is exactly what produces the deep ache and the knot that no amount of stretching seems to clear.

The good news: this is one of the most fixable problems I see. Three changes — pulling the mouse back in, dropping your work surface to elbow height, and giving your arm something to rest on — resolve the large majority of cases. Below are the three causes, the research behind each, and the single product that fixes each one.

Cause #1: You're Reaching for the Mouse (Shoulder Flexion / "Mouse Arm")

This is the one I catch most. The mouse drifts out to the right, past the keyboard, a little farther than it should be. Every click now starts with a small forward-and-out reach. That reach is shoulder flexion, and flexion under load is what fatigues the muscle.

The research here is clean. Gonçalves and colleagues, in the Journal of Physical Therapy Science (2017), measured upper-trapezius and anterior-deltoid activity at shoulder flexion angles from 0° to 60°. When the forearm wasn't supported, muscle activity climbed steadily as the flexion angle increased — the farther you reach, the harder those shoulder muscles work just to hold the position. The fix isn't to push through it. It's to shrink the reach.

The first move is the mouse itself. A lighter mouse that keeps your hand in a neutral handshake position — rather than flat and pronated — lets the whole arm sit closer and calmer, with less of the gripping and clawing that radiates up into the shoulder. Cause #2: Your Desk Is Too High and Your Shoulders Are Shrugging (Shoulder Elevation)

Most desks are built at about 29 inches — a height that suits very few people once a keyboard and mouse sit on top. If the surface is even an inch above your seated elbow height, you can't reach it without quietly lifting your shoulders. People rarely notice they're doing it. They just feel the burning band across the top of the shoulders by lunchtime.

That shrug is sustained upper-trapezius contraction, and it's the same low-level, all-day load that the forearm-support research keeps pointing to. Because your arm weighs what it weighs, an unsupported arm working above elbow height leaves the shoulder holding that weight without a break. You drift forward into a rounded, hunched posture, and your shoulders end up carrying your arms in mid-air all day with nothing to lean on.

Why does my shoulder hurt from the mouse but not from typing?

Because your mouse hand spends far more time isolated and reaching than your typing hands do. Both hands stay home on the keyboard, but the mouse hand repeatedly travels out, hovers, and clicks — small reaches that keep one shoulder loaded while the other rests.
