Just about every week, someone sits down across from me, rubs the base of their spine, and asks the same thing: “If I get a standing desk, will my back stop hurting?” It’s a fair question. Standing desks have been sold for a decade as the cure for the aching, stiff lower back that comes from a full day in a chair.
The honest answer is more interesting than the marketing. The research does show that a sit-stand desk can reduce low back pain for a lot of people — but not for the reason most people assume, and not if you use it the way most people do. Below is a plain-English walk through the actual clinical literature: the in-vivo spine studies, the randomized trials, the systematic reviews, and the inconvenient finding that standing all day can cause back pain too. Every claim here is linked to a peer-reviewed source so you can check it yourself.

You’ve probably heard that sitting puts far more pressure on your spinal discs than standing — the figure usually quoted is something like “40% more.” That number traces back to pioneering disc-pressure work from the 1960s, and it has been quietly revised by better measurements since.
In 1999, researchers implanted a tiny pressure sensor directly into a living, healthy lumbar disc and recorded pressure across a full day of normal activity. Relaxed standing measured about 0.5 megapascals; lying down dropped it to around 0.1. Crucially, they concluded that pressure during sitting may in fact be lower than during erect standing, not dramatically higher, and that a relaxed, slightly reclined sitting posture unloads the disc rather than crushing it.1 So the simple story — sitting bad, standing good, because of disc pressure — doesn’t hold up.
What that same body of work points to instead is the real culprit: staying still. The 1999 study emphasized that constantly changing position is what keeps fluid and nutrients flowing into the disc, which has no direct blood supply of its own.1 A disc is fed by movement. Hold any one posture — sitting or standing — for hours, and you starve it, stiffen the surrounding tissues, and let the postural muscles fatigue into the slumped, end-range positions that actually provoke pain.
This reframes the whole question. The problem with a typical desk day isn’t the chair itself. It’s that the chair makes it effortless to hold one static position for eight hours. A standing desk only helps to the degree that it gets you moving between positions. That distinction runs through every study below.
What the trials found when desk workers switched
Several controlled studies have done the obvious experiment: take office workers, give some of them sit-stand desks, and measure back pain over weeks or months. The signal is reasonably consistent — people tend to hurt less — though the studies are small.
The most directly relevant study is a 2016 randomized trial out of Stanford, published in the Journal of Occupational and Environmental Medicine. Forty-six university employees who already had self-reported chronic low back pain were given access to a sit-stand workstation. Over the follow-up period, those using the workstation reported statistically significant reductions in both their current and their worst low back pain.2 The authors attributed the benefit specifically to postural variation across the day — not to standing as such.
Earlier, the CDC-affiliated “Take-a-Stand” project equipped sedentary workers with a sit-stand device for four weeks. Sitting time fell by roughly 66 minutes per workday, self-reported upper back and neck pain dropped by 54%, and mood improved.3 The detail clinicians remember most: when the devices were removed, nearly all of those gains disappeared within two weeks.3 The benefit was real but entirely dependent on continued use — it was a behavior, not a one-time fix.
A 2018 pilot comparative-effectiveness trial in Applied Ergonomics tested sit-stand workstations with and without an added exercise program in office workers at risk of low back pain, and found the intervention feasible and acceptable, with reductions in low back pain severity.4 It’s a small study, but it points the same direction as the others.
Most recently, a 2025 randomized trial in Applied Ergonomics took 56 desk workers who already had low back pain and tested how they alternated. A fixed schedule — roughly 30 minutes sitting to 15 minutes standing — produced meaningful reductions in both worst and average low back pain over three months, and also improved concentration and reduced presenteeism.5 This matters because it starts to answer the dosing question the earlier studies left open: a structured ratio outperformed simply “standing whenever you feel like it.”
Taken together, these are encouraging, broadly consistent findings. But before you read them as proof, it’s worth seeing what happens when researchers pool all the evidence and grade its quality.
What the big reviews conclude about the evidence
This is where honesty matters more than enthusiasm. When the entire field is pooled and graded, two things become clear: sit-stand desks reliably change behavior, but the certainty around their health benefits is still modest.
The benchmark synthesis is a 2018 Cochrane review of workplace interventions for reducing sitting. Pooling the trials, it found that sit-stand desks — alone or with coaching — cut workplace sitting by roughly 100 minutes per workday in the short term, and by about 57 minutes per day over three-to-twelve-month follow-up.6 Two important caveats came with that: the authors rated the overall quality of evidence as low to very low, mostly because the studies were small and short; and reassuringly, standing more did not produce harmful effects such as increased musculoskeletal pain.6 In other words, the strongest claim we can make with confidence is that these desks get people to sit less — the downstream pain and health benefits are promising but not yet nailed down.
A 2019 scoping review in Applied Ergonomics reached a compatible verdict from a different angle. Reviewing the literature across behavior, discomfort, posture, and health outcomes, it found that sit-stand desks effectively change behaviors and reduce discomfort, but that these changes only mildly affect harder health outcomes.7 The review also flagged a practical blind spot: most participants were given desks with little guidance on how to use them, and the right “dose” likely varies person to person.7
The catch nobody mentions: prolonged standing causes back pain too
Here is the part the marketing leaves out, and the reason I push back when a patient tells me they’ve started standing all day. Prolonged static standing is itself a well-documented trigger of low back pain — even in people who walked in with healthy backs.
In controlled standing studies, a substantial share of previously pain-free adults — commonly around 40%, and across studies anywhere from roughly a third to most participants — develop noticeable low back pain within an hour or two of continuous standing. These individuals are formally called “pain developers.”8 The trigger isn’t standing per se; it’s standing still. The research repeatedly links it to a lack of movement and to specific muscle-control patterns that appear before the pain does.8,9
And this isn’t just transient discomfort to shrug off. A follow-up study found that people who develop this temporary standing-induced back pain are significantly more likely to go on to develop clinical low back pain later — the kind that sends you to a doctor or keeps you off work — with a roughly threefold increase in the odds of a clinical episode over the following two years.9 Transient standing pain is, in effect, an early warning sign.
So replacing “sit all day” with “stand all day” doesn’t solve the problem. It trades one static-loading injury for another, and adds foot, leg, and circulatory complaints on top. This is exactly why the trials that worked — the Stanford study, the 2025 ratio trial — all hinged on alternating, not on standing.
The practical answer: alternate, don’t just stand
Pull the threads together and the evidence points to one clear rule. A height-adjustable desk can absolutely help your low back — but only as a movement tool. The benefit comes from breaking up static loading by shifting between sitting and standing, not from how many hours you spend upright.
Here is what I tell patients, grounded in the studies above:
- Alternate on a schedule, not on a whim. The 2025 trial that worked used a fixed ratio of roughly 30 minutes sitting to 15 minutes standing.5 A 1:1 or 2:1 sit-to-stand rhythm, prompted by a timer, beats “stand when I remember.”
- Cap any single standing bout. Because standing-induced back pain tends to show up within an hour or two,8 sit back down (or walk) before you get there. Continuous standing is just sitting’s mirror image.
- Add micro-movement. Shift weight, step in place, change posture. The disc is fed by motion, not by holding a “perfect” pose.1
- Stand on something forgiving. An anti-fatigue mat encourages the small weight shifts that reduce standing discomfort.
