Most of my patients walk in thinking their neck pain is from stress, or from sleeping wrong one night, or from getting older. In almost every case, I'm back to their desk setup within five minutes. The neck is where the body reports the problem — it's almost never where the problem starts.
The pattern I see most often: the monitor is too low, the chair doesn't make real contact with the upper back, and the person has been holding their head slightly forward for six to eight hours a day, five days a week, for years. None of those things feel dramatic in the moment. Together, they're what's wrecking your neck.
The good news is that the most common causes are fixable — and most of them don't require buying anything. Below are the five causes I see in my clinical practice, in the order I address them.
Your Monitor Is Too Low
This is the first thing I check in every assessment, and it's the problem most of the time. When your screen is below your natural eye line, your head tips forward to meet it — sometimes by 15 or 20 degrees, sometimes more. That shift is the problem.
Your head weighs roughly 10–12 pounds in neutral position. Research published in Surgical Technology International (Hansraj, 2014) calculated that at just 15° of forward flexion the effective load on your cervical spine rises to around 27 pounds. At 30° — a common angle for someone looking at a low monitor — that load approaches 40 pounds. Hold that for eight hours and it's not surprising your neck is screaming by 4pm.
The standard: the top third of your screen should be at or just below your eye level when you're sitting with your back fully against the chair. Most people have their monitor set two to four inches below that. The fix for most people is a monitor arm, which lets you dial height and depth in seconds rather than stacking books and hoping for the best.

Your Chair Has No Real Headrest Contact

- Position keyboard and mouse at elbow height. Your elbows should be at approximately 90° with your shoulders relaxed — not shrugged. If you're reaching forward, bring the keyboard closer.
- Adjust headrest if your chair has one. It should make light contact with the back of your head when you're sitting in your normal working position — not when you're fully reclined. If it only contacts when you lean back, reposition it upward.
- Set a 20–30 minute movement reminder. No ergonomic setup eliminates the need for position breaks. Static holding is a risk factor independent of posture quality. A timer is the simplest intervention that works.
If you wear progressive or bifocal lenses, lower your monitor 2–3 inches below the standard eye-level recommendation. Looking through the reading portion of progressive lenses requires a downward gaze, and the standard height will cause you to tilt your head back to see clearly — which creates its own cervical problem.

Using heat as your first response to acute soreness tends to increase swelling rather than resolve it.
Can neck stretches replace an ergonomic fix?
No — and this is one of the most common mistakes I see. Stretching relieves symptoms temporarily, but if you're returning to the same forward-loaded posture every day, the muscles will re-tighten within hours. Stretches and movement are valuable complements to a corrected setup, not replacements for it. Fix the load first, then stretch to manage residual tension.
What if my pain includes numbness or tingling in my arms?
That changes the picture significantly. Numbness and tingling in the arm or hand from neck pain suggests potential cervical nerve root involvement, which is a clinical issue that requires assessment by a healthcare provider before addressing with ergonomics alone. Please don't try to self-manage radiating symptoms — get evaluated first.
Is a headset safer for my neck than holding a phone?
Yes, substantially. Cradling a phone between your ear and shoulder produces extreme lateral cervical flexion and isometric contraction of the scalenes and upper trapezius — it's one of the most mechanically hostile positions you can hold a phone in. Any headset or earbuds are safer than that pattern. If you're on calls frequently, a dedicated headset also frees both hands, which reduces the secondary arm and shoulder loading that often accompanies phone cradling.
